I'm usually not good with this whole song meaning thing but I'm catching on, either that or Philmont songs are easy to interpret, haha.
Let's get down to business:

If you're going to follow Jesus, you have to leave your old sinful life behind and give ALL of you to him, not just part or little bits. THE WHOLE YOU! And now he's finally saying this time I'm truly going to live my life for you.
By "The Difference" he means that there has to be some difference in his life if he follows the Lord. There's got to be some form of evidence of Christ in his life. When you give and devote your life to Christ, there WILL be a difference and it WILL be significant. :)

I agree with RelientKisrad. I will add my 2 cents worth. If we focus on the line "I've tried so hard to mix the old life with the new, But there's not in between if I'm gonna follow You" and the chorus, it can be broken down into this: When non-christians know you are a christian, they watch you very closely. They ask themselves what is different between you, a christian, and them. If we really have a relationship with Jesus Christ, people should be able to see it through your actions. They should see a significant difference beween you and the rest of the world and if the Lord is in your heart changing your life it should be SO obvious.
