For anyone who might have stumbled into this song. The band I play and I will play this song as a tribute. The whole song is actually about drug addiction. I believe it is heroin but I could be wrong about it. "Evil Twin" is that who you are when you have your fix, to who you are when you need your fix. And talking about dying when you are near is making reference to the inability to stop using and how frustrating it gets.
