For me, this song is about someone who is hurt to the essence of their being by someone who they have loved very deeply. This person loved her more than anyone ever has, so she put up with things that she would not have otherwise.
The heartwrencher is undoubtedly the 15 or so second long vocal run. You can literally FEEL the pain that she feels while telling this person how much they have hurt her.
