At first i thought it was domestic abuse. But now this song seems to be about a woman who is somehow used by a man. Maybe he uses her just for sex and spend the rest of his time with his wife. I don't know.

I am fairly certain that this song is not only about an unfortunate and toxic relationship in which the woman is depressed, devastated, detached and clearly hurt by the relationship and her partner, this is also about having to present yourself to the public as if you were okay.

The woman is hurt, she is in an abusive and unhealthy relationship, but appears to still love the man she is with, lying to herself that everything is okay. The reason why Dio refers to it being a personal situation is because it's something that the couple should be dealing with in a healthy manner and on their own, but the cameras and the public, very likely a reference to the paparazzi and the general public, are obsessed and want to know every single detail. They swarm the woman and/or couple and she has to smile for them, pretend that everything is alright, hence the line "do you look as good as your sister", probably implying that her sister, who is also somewhat of a celebrity, is having a better relationship in the eye of everyone else.

It's a matter of celebrities and anyone who is even remotely known by the public being constantly stalked and their privacy invaded. Just because celebrities usually tend to have a significant financial privilege, that doesn't mean that they aren't people with emotions and problems. One of very frequent topics in the Classic Rock genre is about the problems and societal issues that rockstars face, despite the fact that they're "living the dream", as some might say. Dire Straits wrote Money For Nothing, Metallica wrote Turn The Page, The Ramones wrote I Wanna Be Sedated, Pink Floyd wrote Comfortably Numb, there are many other songs about it. The fact that even the fans are unaware of how gruelling and demanding of a job it is to be a performer is what causes so much harm to their beloved stars, makes them spiral into depression or addiction to drugs or alcohol.
