I don't think it is literally about murder since this appears to take place early in the relationship before the Alpha Couple hits rock bottom. The poison could refer to their alcoholism and perhaps one blaming the other for introducing it into the relationship. Either way, the main character loves the other deeply but also knows early on that the relationship is toxic and doomed.
