I'm not sure, but I always hear "what did I do? What did I do?" Instead of "what could I do? what could I do?". I might be wrong, though.

I think it's simply about someone who fucked up. It's not specified in which way, so everyone can feel free to interpret it how they want. However, it's obvious that the singer's aware of the fact that she messed up and can't forget what she's done, let alone forgive herself (all the things that I've done go around in my head). Obviously, whatever she's done has hurt her girlfriend (again, I interpret that she as her girlfriend, but it might as well be interpreted as a friend, family member etc.) deeply, to the point where her girlfriend tried to hurt her back verbally and maybe even broke up with her (all the things she said). Her girlfriend has lost faith and maybe even some love for the singer because of what she's done, and this seems to be most visible to the singer when she looks into her lovers eyes (in her eyes, it's all there in her eyes). Before she screwed up, her lover would look at her with love and happiness, while now her glance is shallow and despicable. (If you've ever really screwed up and looked into the eyes of the girl you love and who once loved you, you'll probably get what I'm trying to say. Seriously, it's a horrible thing and kind of makes you afraid of looking into her eyes again.)
