I think it's about addiction in general. Nowhere in the song does it specifically state what it's about. Also, just because it makes the relation to a bloody nose, does not mean it's just about coke. Plenty of things can do that. Meth, pills, heroin, anything. I do think it's more so about drug addiction, or something of the sort because "And there I was all cloudy-eyed I was trying to speak. The family coming with concern and all.

I think it's about a relationship that's "off and on, off and on." And he wants to quit it, he wants it to end, but he just doesn't have "the strength to live without it." So he will continue the rollercoaster ride.

I agree with Ellie117. It's definitely about an off-and-on relationship, that hurts both the people in it and those around them (the part about the family), but they continue to live in denial.
