it kinda sounds like the narrator or someone he knows has a terminal illness or something -> talking about a man w time on his hands (which he doesnt have). and how its so real, and he has to carry this burden alone (maybe he hasn't told anyone yet?) and how someone's not ready to go. im probly waaay off but thats just wat i got =)
