I kind of get the vibe that it's about a couple splitting up. I would assume the woman cheated (unless the song is for some reason sung from the woman's perspective) and thats what slipped out causing the doubt. The second verse I gather is about them trying to work it out, but it doesn't get anywhere and the battle to keep their relationship alive only goes uphill. Also in "Forever ending, never done." even though their relationship is over, they have children (implied by the family part later in the song although it's not definite) and they have years of raising their children and with that comes interaction between them that would be a reminder of what happened. Also he would take down the pictures of the family if they split up/divorced. You get the picture.
The last verse I don't really think fits in though so I could be totally wrong.
