I always thought it was, "All we hear is our heart in our ears." The way you can sometimes hear your pulse when you're under stress and it's silent.
This song has been one of my favorites for years. I'm not sure on the exact interpretation of the song, but I know the feelings it gives me. Typically: nostalgia and heartsickness. This song hurts so good, as they say.
