I don't know what it's about, but it seems like it may be about depression and denial of that depression

I love this song though

Like a lot of Flatliner songs, it's fairly obvious, and it's about depression. I don't think the 'decay' part is as much 'denial of depression', but rather about keeping up the facade of normality to others. It's about someone who is in a place of no return; nobody really cares about him and all he's doing right now is 'running on no sleep', like a machine that badly needs to be oiled. It's rejection by something at its finest, and knowing the Flatliners that something is probably society as a whole.
