It's about being cheated on and karma coming back to get the cheater. For Justin, this is clearly a song to britney.
In an interview he said that this song isnt about Britney. Theres rumours going around that its about Trace Ayala and Elisha Cuthbert. Juicy song, love it.

People here are talking about Britney spears. I definitely don't think its about her as she's got a husband and kids at the time. This is inspired by his best friend Trace's ex elisha Cuthbert who cheated on trace with her friend at her house (the music video shows it). "Don't wanna talk about it, don't wanna think about it, iam just so sick about it" is actually sentences from the conversation justin had with his friend after he broke up with elisha. He also confirmed this in an interview i guess. Just because he had a famous and publicized relationship and breakup with britney doesn't mean he's gonna carry it around on his songs guys, get a brain.
of course he's gonna say it's not about britney... but c'mon. after "cry me a river" and its video, being asked so many times if infidelity was the reason he and britney split.... coming out with a song like this on his sophomore album makes it pretty clear. especially with all of the similarities between the lyrics in this song and all of the things he has said regarding his relationship with britney...
when i first heard it, the part where he starts in with "you spend your nights alone"... i just broke out in goosebumps. such an intense song.
