I think about its a person that is a smoker and gets tested positive for cancer but continues to smoke anyway, and eventually dies.
One of the first songs I've heard in a while which is genuinely touching. It's obviously about someone Wade knew who died of cancer and I think the way that the song talks so frankly about it, actually mentioning cancer, chemo, radiation and cigarettes, makes it so much more powerful than a song that obscures its meaning behind metaphors.
