Narakaloka is in hindu mythology a place consisting of 28 different hell equivilants. Of the 28 the lowest one is a place with a lake some 8,000 long filled with worms. You end up in this worm lake for having a small portion of food and not sharing it with company. The song is about a dying man's fear of ending up here.(John @ some concert in chicago around the time it came out) =)

I can remember the first time I listened to this song. I was rocking along with this guy, growing cabbages, making toast. Life was good. Then it got to the line "the doctor says that I've got thirty days left at most". It nearly tore me in half.
