I think the song is definitely about Alzheimers. Probably even the daughter singing to the mother who cant recognise her anymore.
"Say my name So I will know you're back you're here again For a while"
The "for a while" is the slight sudden traces of memory.
And also, near the end of the song,
"You drift away again Too far from where I am When you ask me who I am"
probably means that the mother cant remember her anymore. All the lyrics fit if this was about Alzheimers.
Especially this part:
"You're not aware Your hands keep still You just don't know that I am here"
The song is about a parent / grandparent with Alzheimer's or demetia. It's very personal for Sharon, there are several videos on youtube of live performances where she struggles to sing the song through her tears.

It's very, very obviously about someone dealing with a loved one who has dementia, and struggles to recognize who they are. Look at the chorus. They can occasionally, briefly recall who this person is, but otherwise, they just can't.
This isn't about a failed relationship or anything of that sort. It's something far, far more severe than that.

I love the build up near the end, it's so amazing. Really moving song. I've always thought it's about a man and woman who've known each other since they were little and now after growing up, the guys gone away and left her. She's saying she knows he belongs with her, she knows him too well and their past is too incredible to ignore (Think Cathy and Heathcliff) Actually, saying that, this wrong really reminds me of the relationship Cathy and Heathcliff (Wuthering Heights) do have. Brilliant song : )
I agree. This song also remindes me of the relationship me and my ex boyfriend had... Basiclly the same thing, just we aren't together anymore :P

I think it's about a daughter or a son whose mother or father is already old and in a verry bad condition or had an accident and now are in coma (I don't now if this is the correct word; please correct me if I'm wrong). I think my coma-theory fits best.
I actually listened to this song again. At first I thought it was about a love/ex-boyfriend. But then reading the lyrics I thought of something else. It reminded me of this episode of Roswell I just watched where Liz's grandmother was dying and she asked Max to bring her back but he couldn't but he allowed her to say good bye to her. It made me think that maybe this person is in a coma and she wants them to come back to her again. But then the last part made me think of the Notebook. It's like who ever she is singing to is sick and can't remember her and she just wants them to say her name so that she knows everything will be alright.

These two parts made me think of that. Like in the Notebook he is telling her the story of how they fell in love but she doesn't remember and it's like WT is saying remember who she is and say her name and let her know that you remember everything. And that since the person has Alzhiermer's they can't remember anything and they are too far away from them because they can't remember anything. And how she says crossing the borders of time, it's like saying I'm remembering everything and I want to be with you no matter what. It really just reminds me of the Notebook how all they want is that spilt second where they will remember everything they went through to be with each other. Or not even a relationship wise but just remembering a time in someone's life that is important that might not be able to remember. My grandpa had Alzhiemer and it's like you just want them to be how they were before they got sick.
Reminds me of my grandfather who had Alzheimers. It's a very beautiful song.
I think this song is about a person who's loved one has Alzheimers disease and is dying.

They are asking their loved one to remember their name and to remember who they are, but of course the person has Alzheimers and cannot remember. This really hurts the person, who says
"It hurts to much.. I pray now that soon you're released to where you belong."
I.e. "I love you and you deserve this peace".
I can really relate to this song because my grandad had Alzheimers. It always brings a tear to my eye.
I believe it's about seeing an old friend you haven't seen in years, but they don't recognize you, and that really hurts you cause you used to be best friends. -shrugs- But I get where everyone else is coming from.
I really love when she sings 'I cross the borders of time Leaving today behind to be with you again' and 'Too far from where I am When you ask me who I am' I loved the emotion in it.
