This song's pretty self-explanatory. I think it's about letting go of everything that you keep bottled up inside you, in this case physical abuse.
Maybe this guy realizes that he's been abusing/controlling/taking advantage of someone, both of them are oblivious to it until it dawned on him what was going on.
