I think he and his girlfriend broke up and she treated/ is treating him like crap: "Poured salt on my wounds when my stitches tore". Thats why he says "And now I'm giving it back cause payback's a whore", but to do so he needs the lobotomy, a surgery which may change personality, because he might still have feelings for her I guess. And his new personality "would make it easier to throw it all away". Correct me if I'm wrong =)
