This song is about how people generally don't let those who are important know exactly how much they mean to each other, and how our society doesn't really cultivate that kind of expression.
He is saying that whether or not he says the words himself there are indeed people that are important to him. The imagery of the volcano at the end is passionate, and invokes the intensity of those feelings. The looming threat of the volcano is palpable for the people that live around it regardless of whether or not it erupts. He is expressing how he tries to make the people around him feel as though they are important to him even though he rarely tells them.
