It's about Johnny Thunders of the Heartbreakers and New York Dolls. He's known notoriously for being addicted to smack causing fuckups in his performance. Oh yeah, and Johnny did, unfortunately, die.
I think artists have used the name "Johnny" as an ambiguous term for troubled rockstar any town anywhere as a culmination of Johnny B Goode (known to be covered by J.Hendrix as well) Johnny Cash, Suicide: "Johnny" is the first single by Suicide from their 1977 self-titled album.
