The phrase "Stitched Up" was actually said by the bass player on this song, and is where Mayer got the inspiration for the chorus. He means that he doesn't want to be emotionally wounded by this girl.
He's talking about falling for a girl that he sees from across the room, but knowing that a girl so beautiful will only cause him drama/heartache in the end, and he doesn't want to be stitched up. Her beauty will draw attention from other guys, which would make him jealous and possibly make her cheat. He doesn't want to spend his whole life looking over his should (behind his back) to see if other guys are coming onto her/if she's being unfaithful. One of my guy friends told me once that he would rather date "average-looking" girls rather than the most beautiful girls, because he wouldn't feel so threatened by her cheating on him or by guys wanting to take her away from him.
Also, the chorus makes me think that he himself doesn't want to be trapped in by a relationship with a gorgeous girl, which is a common fear many people have when it comes to relational commitments. Whether she's gorgeous or just average, he isn't ready to be in a relationship that strings him out or locks him down.
Favorite lyric: "I mean girls like that don't sleep alone." That line makes me tingly EVERY time I hear it. Self-explanatory.

This song is obviously about being addicted to Heroin and trying to kick the habit. "stitched up out of my mind strung out lagging behind..." "stitched up feeling the burn, strung out finally learned." When you withdraw from heroin it's like your skin is being ripped off your bones and burns like hell. And also "I've spent my whole life lookin behind my back i just don't think i'm up to that". "Who's to say she's single who's to say she's on her own, girls like that don't sleep alone" meaning it could be laced with something.
In the beginning of the song, it seems as if he doesn't want to be in a relationship, as he says, "Don't wanna be stitched up." However, as the song progresses, it seems that he doesn't want to miss the opportunity to be with this woman of interest because he may always look back to her as being the ideal woman. The chorus then changes to say he is "stitched up." And by the closing of the song, he's begging to be stitched up. He's still conflicted, but insists that he's down to be trapped in with her.
