In think this song is about Carl and/or Kate. People he loves, but feels betrayed by. Kate because people advise her to dump him, and Carl because he usurped him from the Libs.

Whilst the song is not necessarily about drug use - it does reference a famous film about heroin: "The man with the golden arm rarely deals a joker' - A famous film released in 1955 about morphine addiction had the title "The man with the golden arm". This line is a wonderful display of Pete's song writing skills as to most people this would probably go unnoticed.
