This was written after Simon had oral surgery; he was put under an anesthetic called "medazalam" (I think I spelled that right). From what I understand, medazalam makes you all dreamlike, still conscious but very fuzzy, and feeling no pain. The lyrics came from that experience.
This reminds me of a Sylvia Plath poem.

I love that Nick is the lead vocals on this song. I'd love to actually hear him sing though, instead of just speaking. Either way, I love his voice.
