So the peson is slowly dying, emotionally draining and just being pulled deeper in depression, and the only thing that will let them survive is a certain person, who doesn't want to see them again until they die, or in their dreams. He knows she won't care about him anymore or be there for him at all, but he just wants one more kiss from her, apparently it will make everthing alright then.

Since he knows that isn't likely, and he can't hold himself together much longer, well, then he is pretty much saying goodbye and he'll see her in his dreams.
