Well, in my opinion, this song refers to Marijuana, and that's because the entire Low album deals with it. Hail Mary is a song about a guy who's decided that Mary (aka Mary Jane aka Marijuana aka Pot) has ruined his life so now he's taking his last drag before quitting. It's all about double meaning...

Sounds like it's about somebody on his death bed, who's looking back on his life and regretting a lot of his mistakes. Now, in his darkest hour, he's basically praying for one last chance at redemption before his death. The Catholic reference suggests that this song could indirectly be about a real life person Chuck or whoever wrote this song knew (ie. a Catholic who went through what the speaker in the song is going through).
