i think this song means that he's confused about who he is and wants to get back to reality and figure things out... everythings going wrong for him right now.
After I sent this song to a friend of mine who it reminded me of, I was wondering if it was about depression.
Another issue it might be about (probably isn't, but the song still works for this) is abuse. Yes, the "bruises" part is why I'm speculating that it is. The victim keeps "changing [their] mind" about whether to say anything and doesn't know what will help, ie "everything's nothing I need". The bells might be yet another wake-up call that he doesn't know whether to follow or not.
It's a good song, especially if you want to feel down in the dumps or something.

i think its heroin addiction, because the "bruises up my arms" refer to the track marks addicts get from injecting, and the lines "living inside of a dream" seem to mean that numbness that comes with drugs. he's lost and he doesn't know if he should tell someone, but he doesn't even know who he is.
