I think the song is about a woman who tried to kill herself. A friend so happens to come by "stop by for the coffee and cake that you promised" and she is in the kitchen bleeding from her wrists. the narrator of the songs instantly see what’s happening and sees that she needs help. the woman is embarrassed by someone seeing this "saw you messing with your earrings. saw you trying to smile. hey! you don't have to smile for me." and tries to make things seems ok. but they are not.
