I didn't think this was actually about schizophrenia, but rather feeling paranoid in what feels like a crazy world where we're told we're always under surveillance.
Curiously, in the post-Patriot Act America and the Department of Homeland Security apparently encouraging people to report their neighbors for suspicious activity, lines like "the milkman's a spy and the grocer keeps on following me...and the man from Social Security keeps on invading my privacy" seem all the more approrpriate.

this song is eery, cause the band wrote about it decades ago and today in European countries like England, schzophrenia is a problem, its a mental illness thats right up there with depression..
too much drug abuse (lsd mdma meth benzo etc) can lead to what we call acute schizophrenia in anyone. that plus our absurd society goes a lot like this song....
