Everyone seems to get the second "from here" line wrong...it's actually "a juggernaut from here," according to the original hand-written lyrics that are/were hidden at the NewSacredCow website.
Random trivia: there are a few differences between the hand-written lyrics, and what was ultimately recorded. Most notible here is that medicine wasn't originally going to be spelled out. The original version goes:

I think it's about someone who has a problem with substance abuse and their friend clearly sees it and pleads for the friend to stop and look for help.
