I believe this song is about someone who appears to live the high life, such as a rock star, but still as pain they go through life, but continue to hold on to life just as anyone else would do. No one hears them scream because when they are seen in the public eye, they always appear to be happy and content with themselves, when they are really feeling pain deep inside.
