most of the time the things we hate about other people are actually just the things that we hate about ourselves that we see mirrored in them.
i think this song is not only about how thre friends sister is insane but also that everyone is crazy in some sort of way and that it weighs us all down from what we can be/become.

In fact, this is a Skip Spence cover of the song called "Weighted Down (The prison song)" with some verses taken from a song called "Schizophrenia" written by Sonic Youth.
Halmooky is right, half of it isn't in the sonic youth song. The other half is frim Skip Spence. But the way Cat Power combines both songs is great.
