This little piece makes perfect sense if you know that the singer is transsexual (or is it transvestite? I'm hopeless at distinguishing the two, sorry.). In fact, this is one of the most obvious parts by this genius.

the 2.b meaning is the answer. Anna Cantodea is a men with feminine soul.

It's a little sad hear she singing "I wish to cut my genitals and feed them to the dogs" but that's her feeling for herself body.
