A lot of the lyrics point towards heroin, though "the city with no name" is sometimes used as a reference for death. In that case, "she" is the "blade," and she's going to "heal" his wounds by ending his life. I have personal experience with the latter, so that's the interpretation I choose to see. This certainly wouldn't be his first drug song, however.
