it's partly about heroin...

"black sugar shit" - black tar heroin. "burn it black with a candle/all bent at the handle" - referencing a spoon, used to dissolve heroin prior to injection. "devil spit/makes me sick" - possibly written in reference to nausea occurring in those who use opiates with low or no tolerance.

I agree with evrytin strawberrywine said except I think the whole song is about heroin, also i'd like to point out I think- 'Found her stick Filled it thick with... BLACKSUGAR SHIT'- means stick=syringe, filled thick with heroin.xxx

O and also the spoons junkies usta use for injection had to be 'bent at de handle' so they wouldnt burn their fingers heating it up
