I think that it's most likely about the 1981 horror movie "The Evil Dead".
you can tell this is an older Death song because of the lyrics (influenced by horror movies). but I'm not saying the song is bad actually this song is fucking good. great melodic intro and then the song just pounds on relentlessly.
gotta love Chuck's (R.I.P.) screams and yells. raw and brutal stuff.

This song is about a tarot-reader who one day decides she would like to eat a shepherd. She goes to a field in the countryside where she finds an awful lot of sheep. She devises a plan which will capture the attention of the shepherd causing him to return and ergo she can devour him. That night, she creeps down to the field and eats all the sheep. She piles their bones into four piles, one in each corner so the the field has good Feng Shui. The shepherd realises the lady and the sheep's bones and runs into the field. The lady sees her chance to consume the shepherd, but her body is so bloated from the sheep that she cannot approach him to eat him. The shepherd shoots the mutton-swollen lady. Her carcass wears an expression of dissapointment that she will never be able to eat the shepherd. The shepherd gets £40,000,000,000 in compensation and goes on a holiday to Majorca.
