Yes, it is a Nirvana cover. With incorrect lyrics. The original first verse was "I'm on my time with everyone, I have very bad posture"
Actually,not entirely a cover. Courtney Love co-wrote it with Kurt Cobain. The beginning of the song is called,"Just the Same",(a short,unreleased song by Courtney often used as the opening to Pennyroyal Tea when played live).
As far as the meaning goes...there are things that most likely refer to drug use,(heroin causes anemia). I think it's about someone who is just depressed and is looking for comfort. Maybe about someone who feels everyone makes a bigger deal out of them than they deserve.
