It's about having OCD.

David has mentioned in interviews that he used to have obsessive-compulsive tendencies, and that 'the worm' is that constant, nagging need to follow a compulsion. He is 'not in control' of his actions, because the worm is always around. His parents think he's possessed, but he just has obsessions.
