Of course nobody knows for sure except Mr. Rose himself. I think this song, and most of the rest of Chinese Democracy for that matter, is at least in part about the breakup of the original GnR and his resulting isolation and angst.

"You know I tried so hard to make you
Oh to make you change your mind"

According to many sources, not least Slash's autobiography, Axl tried very hard (in vain, ultimately) to make Slash see his vision for what Guns could/should become.

"And it hurts too much to see you
And how you left yourself behind
You know I wouldn't want to be you
Now there's a hell I can't describe"

Axl apparently took rise of Grunge (and the resulting fall of hard rock) very hard, and as a result couldn't handle the idea of Guns going back to being a great little rock and roll band. Slash, Izzy, & Duff didn't see it the same way, in fact Izzy is reported to have left for various reasons, but the recurring theme seems to be that he never wanted to be in the biggest band in the world. He didn't think it was cool. He loved it when they were a great little band, playing clubs like the Troubador and The Roxy. Axl wanted to move Guns in a new direction, further away from their blue-infused rock roots, to try and move with the times and keep the mantle of biggest band in the world. So from his point of view, he wouldn't wanna be Slash, who was happy just to be making music that he thought was cool and touring behind it. To attention hungry Axl, that would be a "hell I can't describe".

"What I thought was beautiful don't live inside of you no more" Axl at least at one time apparently believed that Slash's soul left his body and flew off in a passing crow, on an occasion when Slash overdosed on heroin! Whether or not that was try, seems he's got over it now; Axl, Slash and Duff performed as Guns for the first time since '93 sat the Troubadour on the saturday just gone! I wish he could have gotten over it 20 years ago!

Songs are more complicated that just being about one thing. It's entirely possible that I'm wrong, or that I'm right and some other interpretations here are right too; songs are usually about more than one thing; so this and the rest of the album could indeed be about long lost loves and the breakup of other relationships (such as with Slash and the original band) and lots of other things.
