I think this is about being depressive ("looks like another all time low"), and trying to escape this either through drugs ("you barely even pierced the skin") or sex ("get down on the floor. shut the god damn door") or both. It's a concept similar to the one of "Closer", i.e., trying to escape sadness/hopelessness through activities that lead to physical pleasure.

i think the song is about drugs abuse.
when she sings "everything is echoing", she is talking about the hallucinations caused by the drug she's using, and then, when she asks herself "is this really happening?", she's lost the perception of what is real and what isn't.
and of course, we may talk that there's a will to self destruction, because she says she is "never coming back" but still, in the end of the song, she admits that "i keep coming back again", that is, her purpose in using drugs, her purpose to destroy herself, is useless, because she can't fulfil it.
doubtlessly, this is the best song of their EP.

actually, not only in this song, but everytime Trent uses the word "she" in a song, he's always refering to drugs, never to girls.
we can see a lot of this in songs like "sanctified" (the first song in which he used this female reference to talk about drugs), "reptile" and etc.

In “March of The Pigs”, Reznor sings about people whose greatest goal is to watch you fail, people who are willing for you to fall.
