its about anything .. any sort of addiction or sickness .. its about reflections of growing up in a place where ur choices have defined who you are. in the movie it is about a man having aids and him dealing with being a social outcast.
"walking a thousand miles ..." is talking about tryin to come to terms with what it is you have done choices you have made which make you the person you are today rather then running away from them
