I believe it's about drugs, pretty clearly.. It's all been said by many others, the only thing I think being missed is that the "she" being referred to, is the fix. I've heard many addicts (especially opioid addicts) will refer to their drug of choice as a mistress.. As in, while she's around (while you are keeping up with your habit) everything is great, but when you ignore her, she makes you feel as awful as it's humanly possible to feel.
