I think the song is about a person who is struggling with serious mental problems and their partner who stands by them through time, which makes the person feel like a burden. My interpretation of the song is about a person who is in love but has a hard time dealing with it. He is tormented by the feeling of not knowing whether this feeling is mutual (“if this feeling flows both ways”).
