I think this song is about how the subject hates themself and are constantly “at war” with themself and they find solace in the “guy whos there for them” aka depression/addiction. The depression/addiction is always there, which is comforting, but it only “spectates” and heightens the pain of self hatred. The subject has a mixed hatred not only for themself but for the depression itself as well. “Start with his liver” means he is going to become an alcoholic. “A river of tar” clearly symbolizes smoking, and how the effects of smoking build up tar inside your body, hes smoke so much from addiction that it is like a river. Following to cutting off his fingers is the subject talking about how hes going to stop doing what he loves because it gives him an inflated sense of confidence that he doesnt feel he deserves, he feels he is worthless so he doesnt deserve to feel like a rockstar or be good at guitar. He cuts out his tongue to not only silence himself because he hates what he is and what he does, but also hoping that silencing himself on the outside will silence the depression on the inside. He doesnt feel deserving of a relationship. In the end he asks the listener “what about you” asking them if they also hate themself and feel worthless.
