I think this song is basically the narrator's thoughts while he's trying to get clean and stop doing drugs. I see the Yellow side of "Yellow and Green" as a chronological narrative, where in "Take My Bones Away", the narrator develops a codependent relationship with another drug user (possibly a romantic partner as well), and they try to get clean together. In "March to the Sea", the narrator's friend/partner is unable to cope with the stress of withdrawal and drowns him/herself, and the narrator resolves to make a serious effort to kick his addiction as a result. Then we reach "Little Things", where the narrator has been in rehab for some time and is growing increasingly depressed and moody due to withdrawal. His thoughts during the verses are extremely paranoid and angry, while in the chorus he recognizes these thoughts are just attempts to rationalize continued drug use, and he berates himself for being a "sneaky little thing". By the end of the song, he is somewhat more optimistic, knowing that while he is still suffering, he will ultimately be stronger for it in the end.

I think this song is about going through emotional turmoil, and the point at which one finally begins looking to the future rather than dwelling on things that have caused them pain in the past. The narrator of the song is still upset over what's happened, but he's finally allowing himself to believe in a future where he can process those emotions in a healthy way, even if he isn't quite there yet.
