-I think this song takes place after a break-up or loss of friendship. he's referring to after the dramatic stage of a breakup is over with and his significant other has "moved on." he hopes that she still thinks of him from time to time, when she's alone or least expecting it.

"I know that even with the seams stitched tightly, darling, scars will remain"
-even though they both have seemed to be over the breakup, it still hurts when they start to reminisce or think about it.

"I say we scrape them from each other, darling, and let them wash off in the rain. and when they run into the river, oh no, let the water not complain"
-this could be that he wants the distance between them, that the space will scrape off the "scars" of the relationship/loneliness/breakup and if they start thinking of each other or run into each other, they won't fight it and they will just continue on with their lives.

"I swear that even with the distance slowly wearing out your name, your hands still catch the light the right way, and our hearts still beat the same."
-even though he has accepted the fact that they will never be together again, she will always have a piece of him and he will never be completely over her.

this is just my interpretation of the song. it's a beautiful piece and will always be one of my favorites because of how well it relates to my experiences.
