I think HIV/AIDS is an obvious link... but I think it could also be about any debilitating illness and how one doesn't feel mortal... until you do. Despite all our caution and care, terminal illnesses just seem like something that happens to other people. But once you've heard the words from a doctor, you enter a different state of consciousness and awareness of yourself.

i'm thinking maybe it's about the surreal time after getting dumped by a significant other... when the whole world just feels empty and fake... and it's impossible to get grounded again.

I saw these guys live many years ago at the Roxy on the Sunset Strip.... and they were even better live than on the record. One of my fave concerts ever. I still have the t-shirt ;-)

I always wondered if HIV/AIDS was a sub-theme of this song.

After many sleepless nights ( "unsleepable" ), he gets the bad news from the Dr. "All your friends and your foes Would rather die than have to touch you".

And I think the rest of the song is about reflecting on life up to this point and saying goodbye.

The last part could cynically be seen as 'friends' being relieved when he's done with the debilitating disease, but disappointed when he ends up hanging on for a while.
