I think the part 'walked when I shoulda run' and vice versa is about making wrong decisions or at least making them at the wrong time and now being aware of that. To me it seems it's about a couple that love (or loved) each other and are now separated or torn apart by different worlds. It also might be the other person knows him and wants to get together but he wants to go slowly as to not make a mistake again, and he sees the 'if only's' which don't provide much comfort since he's been alone for some time now. And the other side, his interests seems to not know where he/she is going, being lost and just hoping he/she will find out what is their path and where they belong. And while this is happening the singer notices that time is passing, days going by and they're both living different lives now, being hurt by their situation. It's like they are torn between their past and their future but they don't have the present together. And so the years pass (passed) by and he only hopes they won't regret their decisions because in the end they feel they are connected like blood and bone. I think it's about a difficult relationship full of testing, wrong circumstances and maybe even two people who love each other but don't yet fully know what they want out of the world and out of their relationship.
