I took the song to mean the following:

In the beginning the police are telling him to open the door.
They're shocked by what they find. Screaming.
She's sitting at the piano, emaciated and long since dead.

The woman is a love interest and his mother, that much is clear in my mind,
it's an Oedipus complex and an infatuation it's obvious the man telling the story is totally taken by the female piano player.

His father abused him and his mother watched, she soothed him by playing the piano.

My translation of the last verse "And when MY performance began, she held her breath." This is when he murders her, I don't think he's talking about playing the piano, but his performance of the act of murder.

That's just my opinion, I've listened to the song over 1000 times, I'm not Till so I can't tell you what he was thinking when they wrote it, just what it means to me.

I know this song really speaks to me, makes me feel, and for that alone I am forever grateful for it.

What a powerful piece of music.
