I always thought the song was about someone (probably a soldier) suffering from PTSD after fighting in a war. Just moving through life like a zombie because he's still having nightmares and flashbacks about the war. For him the war hasn't ended. It's still going on in his head.

The first time she sings 'This isn't how it's really meant to be. No, it isn't how it's really meant to be' and she seems fairly convinced that the way things are is wrong. To me, it sounds like more of a statement than a question.

The second time, however, she asks 'Is this how it's really meant to be? Now is it how it's really meant to be?' it appears as if she almost expects the answer to be 'Yes, this is how it's meant to be.'

It's pretty heartbreaking.

If you like songs where a choir joins in during the chorus then I recommend Rilo Kiley's 'With Arms Outstretched.' It's a beautiful song.
