The song is about two women. One is a girl that he is in love with, the other is a prostitute that works on/around La Cienega Boulevard in Los Angeles. In the first verse, he is talking about the girl he's in love with. For one reason or another, he can't have her, and that makes him hurt ("How'd I end up feeling so bad, for such a little girl?" "And I'll hold you close in the back of my mind, feels so good, but damn, it makes me hurt"). He has also gotten to a point of such distress because of this girl, that he's scared of his own feelings for her ("I'm too scared to know how I feel about you now"). The last line of the first verse sums up the relationship he has with the prostitute. He's comparing all of the things the girl does to him, how much she makes him hurt, to what the prostitute does to him; all she does is smile and say "I'll see you around" ("La Cienega just smiles, "see ya around"").

In the second verse, he's talking about both the girl and the prostitute, and how they both affect him in negative ways "And raise my glass 'cause either way I'm dead" "Neither of you really help me to sleep anymore" "One breaks my body and the other breaks my soul" (The prostitute being the one that breaks his body and the girl being the one that breaks his soul))

The song as a whole is about how these two women in his life make him hurt, and how strange it is that the girl hurts him because he is in love with her and can't be with her, but the prostitute hurts him too even though all she does is have sex with him, smile, and wave goodbye.
