I think it's referring to the stages of grief, except they targeted it towards relationships instead of death.

"First, you think the worst is a broken heart"
- This is the despair. The initial moments following the "it's not you it's me" or whatever speeches of the world that are the bane of many relationships. These are the days you cry yours eyes out, or go get wasted, or both.

"What's gonna kill you is the second part"
- This is the denial. Which will kill you. Denying that it happened, that he/she'll come back. Which they don't, and that kills you.

"And the third, is when your world splits down the middle"
- Your world splits down the middle, meaning your entire life changes. and how do you deal with that? on top of the despair, the hang overs, and then the disappointment that the denial causes? you isolate yourself. You don't go out with friends, you don't go out and do the things you like. You stay at home and wallow. Eating fatty comfort foods and maybe skipping a shower for a day or two (which means it's probably a good thing you aren't going out.)

"And fourth, you're gonna think that you fixed yourself"
- you think you're over everything, and that you've put yourself back together in a sense. So maybe you start dating, or clubbing, and hanging out with friends. Cause you're over him, right? Until..

"Fifth, you see them out with someone else"
- You see them out with someone else (maybe your best friends, or that skanky co-worker of his that was always trying to get her claws into him) and you're overcome with jealousy and anger. Which makes you realize you're not over it at all, because if you were, why would you care? Because you still care about this person, duh.

"And the sixth, is when you admit that you may have fucked up a little"
- And then you realize you may have "fucked up a little"
This is the closest stage to Acceptance/Closure, which is the last stage.


But that's just my own interpretation, and it could be flawed.

From watching the music video a few times, it seems to me that the song is mostly about a relationship that is broken apart by a significant others drug addictions.
In the song it could metaphorically be speaking of the S.O. not being there in reference to their addiction changing them into a different person.
What I think is more likely is the addiction drives a wedge into their relationship. The addicted S.O. doesn't originally think they have a problem, and leaves. The other person in the relationship doesn't want them to leave, they care about them despite their addiction and probably just wants to help. But the addicted S.O. doesn't think they need help and pushes them away, and leaves.

"But you can't step on the train"
I think it is literally referring to can't step on the train.
The train is taking the addicted S.O. to rehab, but being addicted, to heroine i believe, they don't want to.

But in the video, in the end the S.O. gets on that train.
And they go to rehab or some kind of support group for those trying to overcome addictions.
